A Life Care Plan is a comprehensive, systematic method to determine the future individual
care requirements and related costs for someone who has long term needs from a catastrophic
injury, accident, chronic illness, or debilitating aging. The development of a Life Care
Plan includes case intake; medical record review with chronological analysis; examination
of supporting documents; client, family and physician interviews; consultation with the
client’s other therapeutic team members; documentation of the client’s care requirements;
and research of equipment sources, vendors, and costs. The Life Care Plan is written specifically
for the client’s geographical area.

A Life Care Planner is a consultant to the legal team, either plaintiff or defendant,
in medical malpractice/personal injury cases such as automobile accidents primarily in
the settlement and/or damages phase. Life Care Planners also are active in the worker’s
compensation field. Their services are available to the general aging population through
case managers, family members, and trustees.
